Rates & Insurance


We accept most major insurance carriers.  However we currently do not accept the following carriers:

  • Cigna                                    

  • Great West

  • Value Options

  • OHP

  • UHC

  • Crime Victims

  • EAP Programs


Services may be covered in full or in part by your health insurance plan.  Please check your mental health outpatient coverage carefully by asking the following questions:

  • Do I have mental health outpatient insurance benefits?

  • What is my mental health deductible and has it been met?

  • How many sessions per year does my health insurance cover, and are there limits on the number of sessions per week and/or month?

  • What is the co-pay or co-insurance amount per therapy session?

  • Is prior approval required from my primary care physician?

Medical services provided by our nurse practitioners may have different deductibles, co-pays, co-insurance and benefit plan structure than your mental health benefits.  Please verify your medical benefits if you are seeing any of our Psychiatric Mental Health Nurses or Certified Pediatric Nurse Practitioners. Verify if you are covered through your Medical Insurance or Mental Health Insurance benefits.


SCS requires a credit card on file to receive services.  For your protection, we will collect your credit card information via the phone and enter it directly into a secure terminal that integrates with our credit card processing company.  We will never keep a written copy of your credit card.  We accept Visa, Mastercard, and Discover.


Copays and self-pay services are due at the time services are provided and your credit card will be charged at the time of service via a credit card on file.  Charges are typically processed within 24-48 hours following your appointment.  Balances for co-insurance and deductibles are due when your insurance carrier processes your claim.

Cancellation Policy 

If you need to cancel or change your appointment, it is your responsibility to call the office at 503-653-0631.  We require at least 2 business days (48 hours) prior to your scheduled appointment for therapy or medication management services and at least 3 business days (72 hours) prior to your scheduled appointment for Testing services.  If you do not attend your scheduled appointment or cancel with less than the required timeframe for the services above, you will be charged based on the following fees:

  • $50 for an individual Group Session

  • $110-120 for a Therapy appointment

  • $150 for a Medical and Prescription appointment

  • $150 for a Testing Intake appointment with less than 3 days (72 business hours) notice

  • $500 for a Testing Assessment with less than 3 days (72 business hours) notice
